Form Builder Add-on for Larapen

Important: this is not a standalone application. It is an add-on for Larapen, which must be purchased and installed first.

Build any form without writing code. The Form Builder add-on gives you a drag-and-drop editor where you assemble fields, arrange them in columns, add validation and show or hide fields based on previous answers. Every form gets its own public page and can be embedded in any page or post with a short tag. Submissions land in an inbox inside the admin panel, notify your team by email, reply to the sender automatically and can be pushed to any external service through signed webhooks.

Contact Forms, Quote Requests, Surveys and More

Use it for contact and enquiry forms, quote requests and project briefs, surveys and feedback, registrations and sign-ups. Conditional logic keeps long forms short by revealing only the questions that apply, and the anti-spam layer (honeypot plus the site CAPTCHA) keeps the inbox clean.

Everything the Form Needs

• Drag-and-Drop Builder: Text, email, phone, number, textarea, select, radio, checkbox, date, file upload, rating, hidden fields and section headings, with per-field validation and widths.
• Conditional Logic: Show or hide fields, or require them, based on other answers, with the same rules enforced on the server.
• File Uploads: Accept attachments with type and size limits, stored privately and downloadable from the inbox.
• Submissions Inbox: Read, mark, search and filter submissions, then export any selection to CSV.
• Notifications: Email alerts to one or more recipients, an auto-reply to the sender, and webhook delivery with signatures, retries and a delivery log.
• Public Page & Embed: Each form has a translatable public page and a short tag to embed it anywhere; forms can be added to any menu.
• Anti-spam: Honeypot field, time check and the site CAPTCHA, all configurable per form.

How It Works

1. Design the Form: Add fields in the builder, set their options and validation, and define the conditional rules.
2. Publish: Share the form's public page, embed it in a page, or link it from a menu.
3. Handle Submissions: Get notified, review submissions in the inbox, export them or forward them to your other tools through webhooks.
